Registering as a Patient
Practice Boundary

Patients wishing to register will be asked to complete a registration form and health questionnaire for each person being registered.  The questionnaire will help us until medical records arrive from the previous doctor.  All new patients will be offered an appointment with the Health Care Assistant for a new patient check.   

If any repeat prescriptions are required a copy of your repeat prescription slip should also be included with your registration information.

Any patient moving out of the practice area will be asked to register with a new practice.



For each patient wishing to register, please: 

1.  Download and complete a Registration form via this link: NHS Inform - Registering with a GP practice

2.  Download and complete a New Patient Questionnaire form :  New Patient Questionnaire

3.  Return the completed forms to the Practice by email to: nhsh.gp55376-admin@nhs.scot or print and post to us if preferred.
