Self-Certification Sick/Fit Notes

Sick/Fit Note for less than 7 days

You do not require a doctor’s sickness certificate for any illness lasting less than seven days.  Your employer may require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.

Sick/Fit Note for more than 7 days

If you are sick for more than seven days, your employer may ask for a ‘Statement of Fitness for Work’.  To request a continuation/extension of an existing sick note please complete and submit the form below.  For a new sick note request please telephone the Practice on 01349 863034 as the Doctor may want you to make an appointment as part of the assessment.

Please note that a ‘Statement for Fitness for Work’ cannot be post-dated, are not considered urgent on-the-day work for the doctors, but we will aim to process any requests within a 48-72 hour period.

Agreement to issue the sick note requested is at the complete discretion of the Doctor.
