Register with this Practice

Patients wishing to register will be asked to complete a registration form and health questionnaire for each person being registered.  The questionnaire will help us until medical records arrive from the previous doctor.  All new patients will be offered an appointment with the Health Care Assistant for a new patient check.

If any repeat prescriptions are required a copy of your repeat prescription slip should also be included with your registration information.

Any patient living or moving out of the practice area (demarcated with a black line shown below on the map) will be asked to register with a practice in their new area.

For each patient wishing to register, please: 

1.  Download and complete a Registration Form

2.  Download and complete a New Patient Questionnaire:

16 and over – New Patient Questionnaire

Under 16 – New Patient Questionnaire

3.  Return the completed forms to the Practice along with proof of ID and address.
