Long Term Condition Reviews – MEDLINK

MEDLINK 

From the week commencing 15th June 2026, Dingwall Medical Group will be using a new system called MEDLINK for reviewing long-term conditions. This system is being used by many GP practices across the country to provide a more efficient and personalised approach to review long-term conditions. 

When you require your annual review, which we aim to be around your birthday month, you will receive an invite containing a link to complete a health questionnaire by email or SMS. If you do not have access to either of these, you will receive an automated voice call asking you to contact the Practice to book an appointment for your review.  

Below are example screen shots of the initial email or text you will receive. When you receive the message please click on the link and complete your online review. This can be completed on a smartphone, tablet or computer. 

Once you have clicked the link you will be given more information about the online review you may need to compete (screenshot below). 

Your responses are saved automatically as you go, so if you can’t complete it in one sitting, you can return at any stage by re-clicking on the link again in your email or text and continue at a later date.  

After completing your review online, you may receive instructions if a physical check appointment (for example, a blood test or blood pressure) is needed. 

When we have all the information needed to assess your conditions, a clinician will then review your online responses and physical tests and decide if a further appointment or discussion with you is required.

• Engage by completing the online questionnaires or calling the practice to book Face to Face reviews 

I can’t use the internet, what do I do? 

If you aren’t able to complete the health questionnaire online, when you receive the text or automated phone call please contact the practice on 01349 863034 and our team will book you into the relevant appointment with a Health Care Assistant or Practice Nurse.
